Just weeks before his passing, Osbourne made an emotional final appearance on July 5, performing seated on a throne during Black Sabbath’s farewell concert in Birmingham — a powerful return to the city where it all began.
Osbourne revealed in 2020 that he had been privately battling Parkinson’s disease since 2003. “It’s not a death sentence,” he said at the time — and he lived by that motto. Despite a litany of health struggles over the years, he continued to perform, create, and inspire.
In February 2025, he announced what would become his last concert — a benefit performance and reunion with Black Sabbath in his hometown. Though no longer able to walk, Ozzy’s voice remained strong. “He’s very happy to be coming back and very emotional about this,” his wife Sharon shared that same month. “Parkinson’s has affected his legs. But his voice is as good as it’s ever been.”
Reflecting on his condition, Ozzy told listeners on his SiriusXM show Ozzy’s Boneyard, “I have made it to 2025. I can’t walk, but you know what I was thinking over the holidays? For all my complaining, I’m still alive.”
Ozzy’s health had been a rollercoaster in recent years. A 2019 fall in his Los Angeles home led to neck surgery. In 2022, he underwent a major spinal operation — one his wife said would “determine the rest of his life.” Yet just two months later, he stunned fans with a surprise performance at the Commonwealth Games in Birmingham, electrifying a crowd of 30,000.
Throughout it all, his passion for music never wavered. “That’s the only thing that reminds me I’m getting older: things going wrong and not working anymore,” he told PEOPLE in 2022. “But I still feel young at heart.”
In 2023, he was forced to cancel a planned U.K. and European tour, citing his ongoing physical limitations despite stem cell treatments and cutting-edge Cybernics therapy. “My singing voice is fine,” he reassured fans, but admitted, “my body is still physically weak.”
Even then, Ozzy never gave up hope. “I never imagined my touring days would have ended this way,” he said — a bittersweet acknowledgment of a career that redefined rock and rebellion.
